Kathleen McInnis, RN, MS was formerly the Care Coordination Services Manager for the San Juan Basin Health Department. She received her BS degree in Nursing in 1986 and began her career as a pediatric nurse in Denver until 1991. After moving to Durango, she received her Master of Science in Community Health Nursing in 1996 from the University of Colorado Health Sciences Center’s School of Nursing. She has worked in school nursing, in the clinic setting, as a theory instructor for a local community college and in legal nurse consulting. Kathleen brings her passion about health and her valuable career experience to SWCAHEC.
Mary coordinates the Asthma/Respiratory Toolkit Program, facilitates the Regional Hub for Connect for Health Colorado, and provides support and outreach for health professional preceptor recruitment. Mary is also the primary care liaison for the Center of Excellence in Care Coordination (CoECC), an 18-month planning grant awarded by the Colorado Health Foundation that facilitates collaboration, shared learning, and practice advancement of care coordinators. The Center provides assistance for the improvement of access to quality, coordinated, and cost-effective healthcare and social services. Mary brings her breadth of experience in the college setting to the healthcare arena, where she has served as an instructor, academic advisor, faculty and staff trainer and mentor, and interim special assistant to the Dean. Mary graduated from State University of New York with a BA, teaching certificate from Buffalo State College, and a Master of Landscape Architecture from State University College of Environmental Science and Forestry.
Karen oversees the Rural Clinical Rotations Program and coordinates community education projects for the general public. The Clinical Rotations Program is a short-term mentoring course which places students pursuing advanced and doctoral degrees into professional practices in rural communities. Karen coordinates housing and identifies service learning projects for students from the University of Colorado Anschutz medical campus and Regis University. Karen also organizes various health education projects that benefit many southwest Colorado communities. Karen is a 1985 graduate of North Carolina State University with a Bachelor of Arts degree in Business Management. She brings her knowledge and expertise of working with non-profits to the coordinator position. Karen is the founding member of the musical group RHYTHYM f/x, and is a Certified Equine and Canine Sports Massage Therapist.
Kerri owns Cardin Communications, a marketing company where she works with client’s across the nation from San Francisco and Colorado to New York and Hawaii. In her role with SWCAHEC, Kerri writes articles and press releases; designs newsletters and ads; works as the special event photographer; and updates the website and social media. Nora Flucke, Project Director
Center of Excellence in Care Coordination
Nora owns and operates the consulting firm Global Care Professionals. In her role at SWCAHEC, Nora performs the advanced quality improvement work for the Center of Excellence and participating care coordination programs.
