Low Vision Problems? SWCI has a program through the AAA for people aged 60+ who are experiencing age-related vision loss (like macular degeneration, cataracts, glaucoma, diabetic retinopathy, etc.). The program can provide up to $500 in financial assistance to purchase assistive technology (like magnifiers, CCTVs, computers and software, special lights) for qualifying people in Archuleta, Dolores, La Plata, Montezuma, and San Juan Counties. Screening with the Ages and Stages Questionnaire (ASQ-3) & Ages and Stages Questionnaire: Social Emotional (ASQ-SE) Training: Saturday, February 10, 2018, 9:00 a.m. - 1:00 p.m. This course instructs the administration, scoring and interpretation of the ASQ-3 and ASQ-SE developmental screening tool. Cost is $10 per person.

San Juan Basin Public Health is hiring a Family Health Care Coordinator. This position performs intensive case management for multiple programs, with a focus on families & pregnant women in our region. The care coordinator will work with clients, community and agency partners to create clear service care plans, promote timely access to care, work to increase utilization of preventative care services. Knowledge of Medicaid, community resources, health programs & critical thinking helpful. A Bachelor’s degree is preferred. The wage for this position is $16.36-$23.72, plus benefits. Although the full salary range for this position is provided, appointments are typically made at or near the range minimum.
