The Regional Health Connector (RHC) for HSR-9 supports practice transformation of practices; serves as a crucial point of connection between primary care practices, other health care providers, public health, and community resources; engages primary care in local community health planning and improvement efforts; and aligns public health and community resources in efforts to transform and improve primary care. The RHC is critical for achieving successful interventions for social determinants of health through partnerships between primary care and community organizations and will support the development of cross-sector partnerships.

Energize Colorado is a nonprofit that brings a diverse professional community together to provide relief to local small businesses, supporting them in recovering from the economic impact of COVID-19.
They mobilize generous Coloradans to provide resources and relief to small businesses, nonprofits, and individuals, supporting them in recovering from the economic impact of COVID-19.
Energize Colorado has announced a grant and loan fund. Grants will be up to $15,000, and loans will be up to $20,000. The funds are for small businesses with fewer than 25 FTEs.

As summer winds down and school is on the horizon, two issues weigh heavily on parents’ minds: What will education look like this fall, and what’s best for my child and family during the coronavirus pandemic?
Children’s Hospital Colorado is helping parents and caregivers make informed decisions based on facts, and they understand that every family’s situation is different. For some families, working from home, virtual learning or homeschooling are viable options. Other families are choosing for kids to return to school for social interaction with peers and in-person learning. And for others, schools play an essential role in children’s access to technology, mental health services, physical exercise and food.

Grant Writing Assistance
Senate Bill 19-228 provided funds to the Center for Prescription Drug Abuse Prevention to assist with grants for prevention, harm reduction intervention, treatment, and recovery of substance use disorders. These funds are intended to aid local government and non-profit organizations with grant writing services when applying for funding to address opioid and other substance use disorders in their communities.

How to Apply
If you have a grant application that you would like assistance with, please submit the brief Grant Writing Assistance Form between July 13, 2020 and December 11, 2020. Grant assistance must be requested at least 3 weeks prior to the submission deadline of the grant, except for small pandemic relief fund applications. Funds are available on an ongoing basis through December 30, 2020 or until all dollars are allocated to grant writing services.

Adolescent Reproductive Health For Primary Care Providers
This series offers clinically relevant topics for primary care providers who provide or are looking to build, expand, and improve care to preteens through young adults. Once completed, participants will be able to evaluate and manage common conditions in adolescent reproductive health. Over the course of six sessions, the series will include an overview of adolescent reproductive health issues, best practices in adolescent contraception and common adolescent gynecology concerns.

Seeking a Regional Health Connector
The Regional Health Connector (RHC) for HSR-9 supports practice transformation of practices; serves as a crucial point of connection between primary care practices, other health care providers, public health, and community resources; engages primary care in local community health planning and improvement efforts; and aligns public health and community resources in efforts to transform and improve primary care. The RHC is critical for achieving successful interventions for social determinants of health through partnerships between primary care and community organizations and will support the development of cross-sector partnerships.

The Regional Health Connector will preferably live in La Plata County and be willing and able to travel independently throughout the assigned region up to 50 percent of the time, including during inclement weather. This position may require some evening and weekend events. *Due to COVID-19, in-person meetings are limited at this time.
|
This is a part-time (approximately 18-20 hours per week), non-exempt position with the potential of increasing to a full-time role and is supervised by the Executive Director. Professional development budget is included. Neither medical nor retirement benefits are provided. This position will be eligible for medical benefits should it increase to full-time. Compensation will be $25.00-$30.00 per hour.
