Together, We Can Increase Wellness in Our Region!
SWCAHEC is committed to understanding issues related to health and wellness in Southwestern Colorado. We have partnered with the Accountable Health Communities Model (AHCM), supported by the Centers for Medicare and Medicaid Services, to support screening for the social and economic barriers to health that impact our communities. Area clinical partners participating in the AHCM project include; Pediatric Partners of the Southwest, Southwest Health System, Axis Health System, and Every Child Pediatrics.

Screening data from patients allow healthcare providers to understand barriers to health in our region and to develop plans with local community organizations to address such things as food security, transportation, housing, utilities, and social isolation. Weekly data reports offer timely feedback to practices and their community partners allowing the development of targeted support services and collaborations. These reports, known as DOMO reports, are always available online and can support grant applications or other programs and initiatives that need to understand current areas of need.

In the coming weeks, we will continue to highlight the most recent data on social and economic barriers to health in Southwestern Colorado. We will also highlight some of the community organizations partnering with our healthcare professionals to address these barriers and provide support and resources to community members.

SWCAHEC offers an AHEC Scholars Program for health professions students interested in supplementing their education by gaining additional experience in rural and/or underserved urban settings. This statewide program focuses on interdisciplinary practice and education (IPE) while bringing a cohort of students together to learn from each other.

Students explore topics beyond the classroom, gain a competitive edge, and learn about emerging issues in healthcare.

Past the Pandemic: Mental Well-being for You and Your Patients
This ECHO series offers health care providers and other staff working in health care settings strategies to navigate, normalize and hold space for worries and experiences during this pandemic. Each session provides helpful, tangible tools in a psychoeducational, didactic format. By understanding stress along a continuum, participants will learn how to manage stress and loss, increase capacity to prevent burnout, elevate connectedness and mindfulness, and promote quality patient care.

Rural Practices Telehealth Stipend - Applications Due October 22, 2020
The “Supporting Providers and Families to Access Telehealth and Distant Care Services for Pediatric Care” project is a grant between the American Academy of Pediatrics (AAP) and the Maternal and Child Health Bureau (MCHB) to support telehealth access and infrastructure for the provision of comprehensive care to children and adolescents, including children and youth with special health care needs (CYSHCN) and other vulnerable pediatric populations, utilizing a medical home approach during and after the COVID-19 pandemic.
|
As part of this grant, the AAP is launching a program that will provide small stipends to practices serving pediatric patients and families. The stipend program will offer $5,000 to community-based practices that serve rural areas (with an emphasis on underserved populations in those areas) and are new to telehealth implementation. The program will support practices with the implementation of telehealth and distance care through participation in a 6-month virtual learning community. Applicants can include practices with non-pediatrician physicians, such as pediatric nurse practitioners and family physicians.

The Lift The Label campaign is actively working to better understand the stigma around opioid addiction specifically in the Black/African American and Hispanic/Latinx communities in Colorado so they can refine communications to more equitably and effectively connect with all Coloradans to remove this stigma. To do this, they are conducting research via phone interviews with people from these communities who have personally experienced opioid use disorder (OUD) to learn about their experiences and give a voice to their community on how they can improve the campaign.
Participants that this research is confidential, and participants are not required to be featured in the Lift The Label campaign. Additionally, they will be compensated for participating in this research.

The Colorado Behavioral Health & Wellness Summit will bring together clinicians, educators, researchers, and policymakers as well as leaders in the field of behavioral health on November 9 through November 12, 2020. The event is a collaboration between the Mental Health Center of Denver, the University of Denver, and Envision: You, an initiative focused on LGBTQ+ behavioral health concerns. Participants will be able to discover new tools and resources, discuss practical strategies for change, and connect with colleagues and experts alike.

This year they will be offering all the content online in a variety of formats including live-streaming as well as video and audio podcasting. Participants will have the opportunity to virtually interact with presenters from anywhere in the world. Issue briefs and resources will be available to download on demand.

We are seeking a care coordinator to screen Medicaid members by phone for social needs (food, housing, transportation, utilities, interpersonal violence) using a standardized assessment.
