Social and economic barriers to health impact the health of many community members in Southwestern Colorado. These barriers include access to healthy food, safe and affordable housing and utilities, and transportation. Social isolation and threats to personal safety also negatively impact health. Clinics in our region are screening their patients about these barriers and using this information to provide more comprehensive care. According to the CDC, Community- Clinical linkages create a network of resource provision that can prevent chronic disease and increase overall mental and physical wellness. SWCAHEC strives to create the linkages that will help address social and economic barriers to health. If you are a clinic with an innovative approach to addressing these issues and/or a community organization that offers services and resources for our region, please contact us at [email protected] to be featured in Pathways, in monthly Lunch and Learns and on our website.
We look forward to increasing wellness in Southwestern Colorado by strengthening the connections between our healthcare providers and those organizations supporting our community members.

Colorado’s dial framework standardizes different levels of “openness” at the county level. It is a tool for counties to use to make life during the pandemic more sustainable, allowing us to balance, to the greatest extent possible, controlling the virus with our social and economic needs.

COVID-19 Fatigue Got You Down? Thursday, November 19
1pm MT | 2pm CT | 3pm ET
We are all tired of COVID, but the hits just keep on coming, and are disproportionately affecting the long-term care space. As we prepare for the challenges of fall and winter, please join us for this interactive Office Hours session that will feature special guest speaker Dr. Lea Watson. Dr. Watson will discuss the importance of recognizing COVID “fatigue” and how to manage the chronic stress it has caused. It is easy to become desensitized to the magnitude of loss, which is dangerous and unhealthy. The antidote is mindful awareness, resilience training and gratitude for the good that is always present. She will also highlight the importance of meeting this moment as one of opportunity to advocate for reform in our industry. In doing so, we can find purpose in a time of chaos and uncertainty. Like no other time, we are all in this together. Dr. Christine LaRocca, Telligen Medical Director, will join the conversation.
Objectives:
Describe three examples of why nursing homes have disproportionate stress during COVID
Discern the difference between acute and chronic stress
List three traits of resilience that can be practiced and cultivated
