Incumbents may act as a lead worker assigning and reviewing work activities, providing input into performance evaluation, and acting as a resource for other staff. This part time position provides administrative support to case managers orchestrating services for the elderly and disable individuals within Archuleta County. Duties also include preparing, organizing and maintaining files, responding to inquiries, referring clients to resources, conducting functional assessments for clients enter a nursing facility, as well as creating individualized service plan, and monitoring client’s services. Additional duties include; entering client information in the state database in a timely manner. The applicant must have excellent organizational skills, be able to multi-task, as well as advocate for clients. A bachelor’s degree in behavioral science is required.
