Specialists conduct inspections, investigate complaints, review plans, prepare reports, and provide community outreach. bachelor's degree in a physical, biological, sanitary, or environmental science and one year of health inspections experience is preferred. The wage for this position is $19.94 - $27.90 an hour plus benefits. Although the full salary range for this position is provided, appointments are typically made at or near the range minimum. Duties include: performing advanced case management duties as assigned for elderly and disabled individuals; managing communications with eligibility technicians; supporting implementation of new initiatives; conducting assessments; developing and implementing case management plans; maintaining documentation in a timely manner; reporting cases for investigation; and ensuring compliance with program requirements. Incumbents may act as a lead worker assigning and reviewing work activities, providing input into performance evaluation, and acting as a resource for other staff. Requirements: bachelor’s degree in a behavioral science and experience in case management within Options for Long Term Care Agency. The wage for this position is $21.04-$30.52 an hour plus benefits. This part time position provides administrative support to case managers orchestrating services for the elderly and disable individuals within Archuleta County. Duties also include preparing, organizing and maintaining files, responding to inquiries, referring clients to resources, conducting functional assessments for clients enter a nursing facility, as well as creating individualized service plan, and monitoring client’s services. Additional duties include; entering client information in the state database in a timely manner. The applicant must have excellent organizational skills, be able to multi-task, as well as advocate for clients. A bachelor’s degree in behavioral science is required.
