San Juan Basin Public Health is hiring a Care Coordination Services Program Manager. The Program Manager provides leadership, direct supervision, & case management guidance to a dynamic team of Care Coordinators & Nurse Navigators, who are working to assist the community with accessing healthcare resources. This position is also responsible for the management of a variety of contracts and grants that are aimed towards community health & resource access. Essential duties of the position are administrative oversight and management of staff & budgets, implementation and monitoring of program deliverables and maintaining community partnerships. Experience in personnel management, direct case management, community resources and health care systems is preferred as well as experience in maternal or child health, public health, or mental health. Bachelor’s degree in human services related field, and 5 years related work experience is required. The pay range for this position is $29.10 to $43.66 an hour plus benefits. Although the full salary range for this position is provided, appointments are typically made at or near the range minimum.
