San Juan Basin Public Health is hiring a Public Health Nurse Home Visitor. This position will provide home visits to women and their families eligible for the Nurse-Family Partnership (NFP) program. Nurses adhere to the NFP model of home visitation and nursing process guidelines, develop therapeutic relationships utilizing concepts of reflection and motivational interviewing, assess physical, emotional, social and environmental needs, goals and outcomes of clients relative to the NFP program, and follow the nursing process in implementing NFP into nursing practice. Applicant must be a Registered Nurse in the state of Colorado, BSN required. Minimum of two years’ experience in maternal/child health or public health is required. Applicants must have CPR certification. The wage range for this position is $24.51 to $36.77 per hour, plus benefits. Although the full salary range for this position is provided, appointments are typically made at or near the range minimum. San Juan Basin Public Health is hiring a Health Equity Coordinator to implement the health equity work of the Equity in Emergency Preparedness (EEP) project, including overseeing the implementation of project activities and coordination with external partner agencies. EEP will ensure that all people living in Archuleta and La Plata counties have equitable opportunities to prepare for and respond to health events, natural disasters, and man-made emergencies, irrespective of socio-economic attainment, primary language spoken, or disability status. Collaborating with traditionally underrepresented and under-served populations to create resilient, connected communities will support population health and prevent future exacerbation of poor health outcomes stemming from emergencies. Bilingual English-Spanish and an associate’s degree and two years of healthcare, public health, or related experience is preferred. The pay range for this position is $17.80 to $25.81 an hour plus benefits. Although the full salary range for this position is provided, appointments are typically made at or near the range minimum. The Program Manager provides leadership, direct supervision, & case management guidance to a dynamic team of Care Coordinators & Nurse Navigators, who are working to assist the community with accessing healthcare resources. This position is also responsible for the management of a variety of contracts and grants that are aimed towards community health & resource access. Essential duties of the position are administrative oversight and management of staff & budgets, implementation and monitoring of program deliverables and maintaining community partnerships. Experience in personnel management, direct case management, community resources and health care systems is preferred as well as experience in maternal or child health, public health, or mental health. Bachelor’s degree in human services related field, and 5 years related work experience is required. The pay range for this position is $29.10 to $43.66 an hour plus benefits.
