A study by Murphy et al. (2017) tested the outcomes of a multidisciplinary care coordination program with the mission to reduce unneeded ED visits while ensuring that patients get the care they need in the appropriate setting. At the heart of the program was an individualized care plan for each participant developed jointly by a team of the patient's health care providers. The team included the patient's primary care physician, emergency physicians and nurses, and mental health and substance-use-disorder professionals, among others. Additionally, the program used a Web-based information exchange system capable of sharing relevant patient information with ED care providers and others, such as case managers and primary care providers, in real time, whenever an enrolled patient registered at an ED using the system. Patients enrolled in the program showed a 34% reduction in ED visits associated with an 80% decrease in the odds of receiving an opioid prescription comparative to the control arm participants of the study.

San Juan Basin Public Health is hiring a Communities That Care Coordinator. This full-time position is responsible for overseeing a local community coalition aimed at preventing youth problem behaviors across La Plata County. The CTC Coordinator is tasked with using effective group facilitation skills to help guide the coalition through the CTC planning and implementation process. Duties also include facilitating meetings, recruiting and engaging key leaders and stakeholders from diverse sectors, data collection, assisting in writing grant proposals and reports, collaborating with partners and stakeholders to provide timely education, and communication regarding program initiatives. The applicant must have excellent communication skills, familiarity with local community, group facilitation skills, demonstrated ability to speak effectively and professionally in public to a variety of audiences, and demonstrated ability to form relationships with strategic partners. This position requires an Associate’s degree and two years of healthcare, public health or related experience. Bachelor’s degree is preferred. The wage for this position is $17.80-$25.81, plus benefits. Although the full salary range for this position is provided, appointments are typically made at or near the range minimum. This part time position will work within the immunization clinic to assess the immunization status of clients, educate, and administer vaccines to adults and children. This public health nurse will also work within SJBPH communicable disease program to monitor disease surveillance data and investigate disease cases as back up to our Regional Epidemiologist. This position requires being a Registered Nurse in the state of Colorado, CPR Certification and one year of nursing experience. The wage for this position is $24.51-$36.77.
