San Juan Basin Public Health Urges Parents to Start COVID-19 Vaccination for Adolescents (12+) to Provide Full Inoculation for the New School Year
San Juan Basin Public Health (SJBPH) urges parents to start the COVID-19 vaccine series for their adolescents soon, in anticipation of the 2021-2022 school year. In La Plata County, 43% of adolescents (12-17 years) have received at least one dose of the Pfizer vaccine series, falling shortly behind the Colorado average of 45% for 12 to 17-year-olds. Archuleta County lags further behind the State, with 26% of eligible adolescents vaccinated with their first dose of COVID-19 vaccine. Vaccination will be crucial to prevent outbreaks and provide protection during the school year.

This 8-week virtual course includes both online coursework (reading, quizzes, assignments) as well as Live Learning Sessions via Zoom web conferencing. Participants are expected to complete assignments each week prior to attending the live learning session. These interactive sessions are an opportunity for participants to connect with fellow navigators and our expert instructors to apply the concepts to their daily work.

A certificate of completion will be awarded upon successful completion of the course.
|
Please note: The time commitment is estimated to be 2-3 hours per week, but may be more or less. It is recommended to have permission from your supervisor to use work time to complete coursework. Attendance at the live learning sessions and completion of 80 percent of course assignments are required to receive a certificate of completion.

San Juan Basin Public Health is hiring a COVID-19 Testing Coordinator, who assists, plans, operates and coordinates communicable disease testing sites in cooperation with local community and healthcare partners. This position will maintain alternative disease surveillance systems and assist with communicable disease investigations in coordination with the Regional Epidemiologist. The communicable disease coordinator will assist with COVID-19 testing operations and trainings. The position will conduct and assist with timely and complete case, outbreak, and incident investigations and implement disease control measures. The Communicable Disease Coordinator demonstrates leadership qualities and may supervise a small number of staff on either a project or an on-going basis, take a lead role in one or more areas of communicable disease or emergency preparedness subject areas, and work independently to achieve program goals. This position will also cross train to assist with vaccine outreach and clinic operations. The qualifications for this position are a Bachelor's degree in nursing, social work, human services, public health, environmental science, microbiology, or related field and five years of human services or public health programs related experience or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job such as those listed above, unless otherwise subject to any other requirements set forth in law or regulation. Benefits for exempt employees and non-exempt employees, 80% or more, are Medical Insurance, Dental Insurance, Vision Insurance, Flexible Spending Account, Employee Assistance Program, Wellness Program, Paid Time Off, Retirement Plan, Life Insurance and Supplemental Insurance. This position is scheduled to close 7/30/2021, or until an adequate pool is reached. The hiring range for this position is $24.51 to $30.64 an hour. Behavioral Health Care Coordinator (BHCC) – Full Time
|
The Behavioral Health Care Coordinator (BHCC) links people in San Miguel County to available behavioral health treatment and mental wellness promotion resources. At the same time, the coordinator advocates on behalf of clients, helps clients navigate the behavioral health system and resources, and coordinates referrals to ensure a streamlined experience for clients. When barriers to care are experienced, the BHCC will assist clients in overcoming obstacles faced in accessing or receiving services needed to maintain well-being and independence through direct services or referrals to other community resources. The BHCC will also provide cross-agency and systems coordination, partner with various behavioral health resources throughout the region, and develop and implement strategies to effectively market and advertise these resources.

The Care Coordinator is responsible for increasing community and partner knowledge of Tri-County Health Network’s (TCHNetwork) programs and services, building strategic relationships to identify and address barriers to good health, and conducting a variety of outreach activities. The Care Coordinator will also provide direct services to clients including (1) conducting both phone and in-home outreach and assessments for individuals enrolled in Medicaid; (2) performing in-home assessments and connecting adults age 60 plus and adults with disabilities to community resources; and (3) identifying client barriers to health and connect clients to/enroll clients in relevant programs to promote health.
