The Care Coordination Collaborative will be meeting on Wednesday December 16th at 3:30 to begin a conversation about the social needs screenings happening in the region and how we can use the AHCM data collected to better inform our work in addressing social and economic barriers of health in our region.
|
|
SWCAHEC will share the data with you from the AHCM program and begin to identify where gaps in community resources exist and how best to address those gaps. We hope we can continue to support care coordinators by providing this opportunity to share, collaborate and partner.

Using Care Coordination to Address
Social and Economic Barriers to Health
|

Current data shows that the majority of patients screened for social needs use navigation services, or care coordination, to meet their healthcare needs. Care Coordination is the organization of patient and client care between partners to deliver the most comprehensive and effective care. In Southwest Colorado, care coordinators work with healthcare providers and community organizations to address specific patient needs, to prevent disease and to increase wellness. SWCAHEC hosts the Care Coordination Collaborative which convenes those involved in care coordination in the region to discuss resources and services, barriers to care, and training needs. Community Compassion Outreach needs volunteers to donate one home-cooked food item every Saturday for 20 Grab-N-Go Bags distributed to the homeless and food-insecure.
The Coffee & Conversations with the Homeless is held at the front entrance of Durango’s Schneider Park on Roosa Avenue, north of 9th Street Bridge. Individually packaged donations are accepted between Noon and 12:15pm. Two masked volunteers remain to sort donated food in plastic grocery bags to hand out with a bottle of water for 20 individuals who come by our station weekly. Sometimes we have only three items to distribute in each bag when volunteers are not in town. At other times, we may distribute an 8–course meal with desserts.

Health Equity: A Guide for Public Health Practitioners (Jan. 20th - Mar. 10th)
This course is for public health practitioners who are serious about walking their health equity talk but aren't quite sure how to get started. Participants will consider how to close gaps that result from systemic, avoidable, and unjust policies and practices that create barriers. This course includes on-you-own work, complimented by weekly, live, interactive Zoom sessions.

Interested candidates must be a strong collaborator with experience in volunteer programming, including recruiting, growing, and organizing volunteers. The Spanish-speaking Food Bank Coordinator must have prior experience with logistics, planning, and program management and successful experience in community engagement and outreach to underserved populations. The ideal candidate will have lived or worked in a rural community for 2+ years, have knowledge of food insecurity and food systems, and have experience as a facilitator or consultant. She/he/they must have a bachelor’s degree or 4 years relevant experience. Ability and willingness to travel locally with reliable transportation. This position is responsible for developing a team to assist clients in accessing health equity programs; identifying needs and generating referrals to community services; conducting social determinants of health assessments; supporting community programming; assisting with insurance enrollments; completing applications for financial assistance. The supervisor will be responsible for overseeing day-to-day operations; developing the field team; establishing relationships with stakeholders; promoting programs; improving staff productivity; increasing client participation; and ensuring program sustainability.

OPEN JOB SUMMARY: Community Mobilizer Administrator
|

The Community Mobilizer is responsible for leading youth and adult community coalitions and workgroups, currently focused on youth substance prevention, in Telluride and surrounding communities. Utilizing the Community That Cares framework and through partnership with local schools, the Mobilizer will lead the implementation of a community youth substance prevention action plan, developing membership in coalition work, and build relationships with community leaders, government officials, educators and other key stakeholders to support the coalition’s goals. Responsibilities also include advocating for community-driven practices and emphasizing capacity building of coalition and community members and engaging local youth to support, grow & lead community initiatives to engage the overall community toward shared goals around youth substance prevention.
