Need Comparison with all AHCM Screenings Completed Region-wide
Screenings for social and economic barriers to health have revealed that our regional community members are struggling with issues related to food, housing, transportation, social isolation, and safety. Compared to other screening regions in Colorado, Southwestern Colorado is showing higher levels of need. SWCAHEC, along with our partners, is using this social needs screening data to create programs to address these needs. As we head in to a new year, we are inspired to continue to work on behalf of our region to address social and economic barriers to health and to increase wellness.
Let’s make 2021 healthier for everyone in Southwestern Colorado!
