We want to hear from you!

Every 5 years, San Juan Basin Public Health (SJBPH) determines the health and wellness needs facing Archuleta and La Plata counties. SJBPH is gathering data and feedback from diverse members of our communities that will help build plans for addressing the top health needs in our region. This survey is an opportunity for us to hear from you about your health priorities and what issues you feel need to be addressed in your community.

This survey is open to any Archuleta or La Plata County resident over 18 years of age, and should take less than 15 minutes to complete. All responses are anonymous. If you prefer a paper survey, these are available at the SJBPH front desk. Submissions of the survey will be accepted through April 6th, 2018.

Survey is here, available in English and Spanish:

If you have any questions, please contact Kristin Pulatie, Director of Assessment and Planning, by email at kpulatie@sjbpublichealth.org or Shannon Shropshire, Assessment and Planning Specialist, by email at sshropshire@sjbpublichealth.org.

Health Equity Messaging and Communications

The Colorado Trust seeks one or more qualified consultants to assist with revisiting and updating communications messaging surrounding the foundation’s health equity vision. This messaging will be used primarily by Colorado Trust staff to explain health equity and the work of the foundation. The messaging will also be provided to grantees and other stakeholders for consideration, to help educate, inform and build upon their own understanding of health equity. The primary goal is to better explain the foundation’s core concepts—including health equity and its relationship to the social determinants of health—in simpler, more intuitive ways to diverse audiences.
Click here to download the full RFP. The deadline to submit proposals is April 6, 2018. For more information, please contact us.

Evaluator Sought for Health Equity Learning Series

The Trust is looking for qualified evaluators who are willing to partner with our Health Equity Learning Series (HELS) team. We want to explore new ways of evaluating how participants evolve in their understanding of oppressive systems, and how these systems are fundamentally linked to health inequities and health equity solutions in Colorado and beyond. Together with the HELS team, the evaluator/evaluation team will design and conduct an evaluation that will help to understand the impacts of this strategy, which aims to increase participants' knowledge and awareness of the causes of inequities.
