Addressing Social Determinants of Health
Applications for March dates opening November 21, 2022
Upcoming Dates: 3/7/2023 – 5/30/2023 and
3/9/2023 – 6/1/2023
Work for this course will be completed in teams of 2 or 3 people.
Together, teams will take their understanding of social determinants of health to the next level as they identify actionable steps that impact the root causes of poor health outcomes in their communities.

With generous support from the Commonwealth Fund and a contribution from the Kaiser Permanente National Community Benefit Fund at the East Bay Community Foundation, IHI invites health care organizations to join a 12-month Learning Community to accelerate progress towards lowering the carbon footprint, or decarbonizing, health care. The climate emergency is the single greatest public health challenge of the 21st century, with the health care sector a significant contributor to greenhouse gas (GHG) emissions.

Newcomer Health
This ECHO series is designed to increase medical providers' knowledge of the resettlement and health issues of newcomers, including refugee, immigrant and migrant (RIM) populations. It will review resettlement pathways, evidence-based screening recommendations, and more common diagnoses and treatment approaches for pediatric and adult populations.
Sessions include brief didactic presentations by immigrant health experts and discussion of participant-submitted cases. Participants are highly encouraged to submit de-identified patient cases for group discussion and expert consultation.
Medical providers across the U.S. who provide care or who want to learn more about providing care for newcomers, including refugees, immigrants and migrants

|
|
The program manager oversees a strategic grantmaking portfolio supporting organizations including nonprofits, community groups and public institutions working in the foundation’s determined program areas. The portfolio includes grants to support place- or issue-based initiatives, including multiyear grants that support the foundation’s vision of achieving health equity for all Coloradans. The program manager oversees resources dedicated to strategic grantmaking, administrative and technical assistance, and relationship management to meet the foundation’s goals. This position oversees program development, seeks grants and proposals, manages budgets and projects, and reviews grantee reports. The program manager collaborates with peer program managers, foundation colleagues, senior leadership and board members to ensure the foundation's activities and programs align with organizational objectives and the mission of advancing the health and well-being of the people of Colorado. The deadline to apply for this position is 5 p.m. MST on Tuesday, Nov. 22, 2022.
