Current data from AHCM screenings in Southwestern Colorado shows that certain racial and ethnic groups are disproportionately affected by social and economic barriers to health, reflecting the larger national picture and further reinforcing the need for action related to health equity and addressing health disparities. This data demonstrates that we have work to do to address the many social and systemic barriers that keep some of our community members from the health and wellness that they deserve. SWCAHEC is joining other non-profits in the state of Colorado to recognize and address social and economic barriers to health by supporting clinical and community linkages, by providing training and education around care coordination, and by promoting community programs whose efforts improve the lives of community members in our region.
