The Care Coordination Collaborative Meeting

As an agency, SWCAHEC recognizes the vital role that you play in increasing the wellness of Southwestern Colorado. You generously collaborate with healthcare organizations, medical practices, and community organizations to provide the most comprehensive care to patients and clients in our region. As a capacity building organization, we would like to support you by providing high quality and meaningful training and coaching opportunities. To this end, we would like to convene a Care Coordination Collaborative to provide the opportunity to discuss what kinds of continuing education will most effectively facilitate your practice. These meetings will allow you to discuss the issues that you face in providing care, and to brainstorm about opportunities that provide knowledge and strategies to address these issues.
The first Care Coordination Collaborative Meeting will be held on March 4th at 3:30 PM. Our meeting will be both in person, and via ZOOM. Our SWCAHEC office is located at 701 Camino del Rio in the Commons Building in suite 320. White House Office of National Drug Control Policy (ONDCP) Director Jim Carroll announced the Fiscal Year (FY) 2020 Drug-Free Communities (DFC) Support Program’s Notice of Funding Opportunity Announcement (FOA), which begins this year’s process of awarding funds to community coalitions focused on preventing youth substance use.

For FY 2020, this will allow 200 new communities to receive direct financial help totaling $25 million from the White House to prevent illicit drug use and save lives. Read on...

As part of HRSA's efforts to combat the nation's opioid crisis, special funding has been provided to increase evidence-based care to patients in need of opioid use disorder and behavioral health treatment. The program offers loan repayment to registered nurses, advanced practice registered nurses, and nursing faculty in exchange for a two-year service commitment at a health care facility with a critical shortage of nurses, or, for nurse faculty, an eligible school of nursing. Now through March 12, 2020.

Each year, NICWA hosts the largest national gathering on American Indian and Alaska Native (AI/AN) child advocacy issues. We are one of the 10 Regional Public Health Training Centers designated by the Health Resources & Services Administration (HRSA) to provide training to professionals addressing public health issues. We serve Region VIII – which includes Colorado, Montana, North and South Dakota, Utah, and Wyoming.
RM-PHTC works in partnership with subject matter experts to develop a skilled workforce, prepared to address public health needs in their communities. Our expertise in adult learning theory and instructional design ensures engaging, relevant trainings.

School Nurse at Mountain Middle School


Regionally competitive annual salary with full healthcare benefits. The position is 180 days a year
following our school calendar with Summer off as well as all School breaks. The Nurse shall meet the following requirements:

BS Degree in Nursing (BSN)

Current license and certification required by the State of Colorado to practice as a Registered Nurse
and​ ​Colorado Department of Education School Nurse; Certification upon employment or within 12
months of employment if new graduate or

A registered nurse who completed RN training in an associate degree program and must be enrolled
in an approved BSN program and provide evidence of continuing enrollment in the program which will meet the requirements for a school nurse and must complete the program within three years. While in the BSN program you must obtain a TEE authorization through the Colorado Department of Education under the supervision of a Colorado professionally licensed person in the same discipline

Current Basic Life Support certification and knowledge of AED use
