Low Vision Problems? SWCI has a program through the AAA for people aged 60+ who are experiencing age-related vision loss (like macular degeneration, cataracts, glaucoma, diabetic retinopathy, etc.). The program can provide up to $500 in financial assistance to purchase assistive technology (like magnifiers, CCTVs, computers and software, special lights) for qualifying people in Archuleta, Dolores, La Plata, Montezuma, and San Juan Counties. Screening with the Ages and Stages Questionnaire (ASQ-3) & Ages and Stages Questionnaire: Social Emotional (ASQ-SE) Training: Saturday, February 10, 2018, 9:00 a.m. - 1:00 p.m. This course instructs the administration, scoring and interpretation of the ASQ-3 and ASQ-SE developmental screening tool. Cost is $10 per person.

San Juan Basin Public Health is hiring a Programs Case Manager. This position provides case management services to elderly and disabled individuals within La Plata and San Juan Counties. Case Manager is responsible for conducting functional assessments, developing individualized service plans, nursing facility assessments, monitoring client's services and needs, adapting services to meet changing needs of clients through ongoing case management. In addition, the Programs Case Manager must input client information in the state database in a timely manner. The applicant must have excellent organizational skills, be able to multi-task, as well as advocate for clients. A Bachelor’s degree in Behavioral Science is required. The wage for this position is $19.24-$27.90, plus benefits. Although the full salary range for this position is provided, appointments are typically made at or near the range minimum.
