Post-Acute Care Collaborative: Your Role Today in Safer Care Transitions

Post-acute care transitions play a critical role in the next steps for COVID-19 care and infection prevention. Whether discharged from a hospital, social distancing at home (sheltering-in-place), or returning to a nursing home facility there are still so many unanswered questions about how to care for and support patients and families during this challenging time.

Our Post-Acute Care Collaborative Office Hours will feature COVID-19 and Infection Prevention speakers from across the care continuum including subject matter experts, local stakeholders, and partners from your state.

The Distance Learning and Telemedicine program helps rural communities use the unique capabilities of telecommunications to connect to each other and to the world, overcoming the effects of remoteness and low population density. For example, this program can link teachers and medical service providers in one area to students and patients in another.

BIGG ELIMINATION TRIBUTE PROJECT

FREE, online, self-paced 7-module series has been developed to help establish replicable frameworks for HCV prevention & education using harm reduction strategies. The overall goal is to build attendees' capacity, confidence, and knowledge to integrate responsive hepatitis C (HCV) screening, testing, linkage-to-care/cure, and support programs into existing infrastructures. The training follows the hepatitis C cure cascade.

PEDIATRIC PSYCHIATRY IN PRIMARY CARE: BEYOND CORE ESSENTIALS

CoPPCAP, the Colorado Pediatric Psychiatry Consultation & Access Program, is offering this free, eight-session ECHO series to give participants the baseline knowledge and skills needed to assess and treat pediatric mental/behavioral health conditions in primary care. Topics covered include digital technology/digital health, aggression and antipsychotic medications, natural therapies, early childhood, disruptive behaviors in preschoolers and school age children, suicide, substance use, and substance use disorder.

AUDIENCE: Prescribing primary care providers working in Colorado
