Family Health Care Coordinator

The Family Health Care Coordinator helps clients navigate the Medicaid/CHP+ systems, as well as non-Medicaid community systems. This includes outreach serving La Plata, Archuleta, San Juan, Montezuma and Dolores County citizens who would like to access public programs and those who are already eligible for public programs, for those under the age of 21 or pregnant. Through Health First Colorado’s (Medicaid) we connect families and pregnant women with resources to obtain treatment for medical, dental, vision, mental health, and developmental problems such as well-child visits, diagnosis and treatment services.
This position is scheduled to close 9/20/2019, or until an adequate pool is reached. The hiring range for this position is $17.80 to $21.81. Care Coordination Manager

Full-Time. The Care Coordination Manager initiatives include care coordination, palliative medicine, social determinants of health assessments, and senior services. The position will manage day-to-day operations for multiple TCHNetwork Community Outreach Programs across our region. The Community Programs Manager initiatives include entitlement and enrollment outreach and our virtual dental home. This position will supervise, hire, train, and conduct regular team and one-on-one meetings, employee engagement and satisfaction, and termination, as well as evaluate and monitor evolving community needs and revise programs when necessary. Interested candidates must possess three years of experience managing a 5+ person team and either a bachelor’s degree or four years of relevant experience. We are looking for a strong communicator who can work with and motivate people of diverse backgrounds and experience levels and someone who can manage multiple projects simultaneously. Access to reliable transportation is required.
