Level 1: Health Navigation Fundamentals Online Fall 2021

Live Learning Sessions will be held on Wednesdays from 11 am-noon MST

Topics in this comprehensive “Level 1” training include patient navigation history and role, patient communication, health promotion and professional conduct. Don‘t miss this opportunity to network with other health navigators!

This 8-week virtual course includes both online coursework (reading, quizzes, assignments) as well as Live Learning Sessions via Zoom web conferencing. Participants are expected to complete assignments each week prior to attending the live learning session. These interactive sessions are an opportunity for participants to connect with fellow navigators and our expert instructors to apply the concepts to their daily work.

A certificate of completion will be awarded upon successful completion of the course.

Please note: The time commitment is estimated to be 2-3 hours per week, but may be more or less. It is recommended to have permission from your supervisor to use work time to complete coursework. Attendance at the live learning sessions and completion of 80 percent of course assignments are required to receive a certificate of completion.

What is the program?

The purpose of this program is to provide support to entities providing health care, health care providers associations, and Federally Qualified Health Centers (FQHCs), taking into consideration the needs of rural and medically underserved communities, to establish, enhance, or expand evidence informed or evidenced-based programs or protocols to promote resilience, mental health, and wellness among their providers, other personnel, and members, collectively known as the “Health Workforce.”

Endocrinology and Diabetes for Adult Primary Care


This comprehensive weekly ECHO series will cover endocrine and diabetes issues commonly encountered in the adult primary care setting. The series will cover a wide array of topics including new developments in Type 2 Diabetes, managing insulin therapy, anti-obesity pharmacotherapy, hyperthyroidism, hypercalcemia management, osteoporosis and more. The Behavioral Health Care Coordinator (BHCC) links people in San Miguel County to available behavioral health treatment and mental wellness promotion resources. At the same time, the coordinator advocates on behalf of clients, helps clients navigate the behavioral health system and resources, and coordinates referrals to ensure a streamlined experience for clients. When barriers to care are experienced, the BHCC will assist clients in overcoming obstacles faced in accessing or receiving services needed to maintain well-being and independence through direct services or referrals to other community resources.
