We want to hear from you!

Every 5 years, San Juan Basin Public Health (SJBPH) determines the health and wellness needs facing Archuleta and La Plata counties. SJBPH is gathering data and feedback from diverse members of our communities that will help build plans for addressing the top health needs in our region. This survey is an opportunity for us to hear from you about your health priorities and what issues you feel need to be addressed in your community.

This survey is open to any Archuleta or La Plata County resident over 18 years of age, and should take less than 15 minutes to complete. All responses are anonymous. If you prefer a paper survey, these are available at the SJBPH front desk. Submissions of the survey will be accepted through April 6th, 2018.

Survey is here, available in English and Spanish:

If you have any questions, please contact Kristin Pulatie, Director of Assessment and Planning, by email at [email protected] or Shannon Shropshire, Assessment and Planning Specialist, by email at [email protected].

Evaluator Sought for
Health Equity Learning Series

The Trust is looking for qualified evaluators who are willing to partner with our Health Equity Learning Series (HELS) team. We want to explore new ways of evaluating how participants evolve in their understanding of oppressive systems, and how these systems are fundamentally linked to health inequities and health equity solutions in Colorado and beyond. Together with the HELS team, the evaluator/evaluation team will design and conduct an evaluation that will help to understand the impacts of this strategy, which aims to increase participants' knowledge and awareness of the causes of inequities.
