Screenings for social and economic barriers to health have revealed that our regional community members are struggling with issues related to food, housing, transportation, social isolation, and safety. SWCAHEC, along with our partners, is using this social needs screening data to create programs to address these needs. As we head in to a new year, we are inspired to continue to work on behalf of our region to address social and economic barriers to health and to increase wellness.

Community Volunteers Launch "CoronaDoc" Information Program

San Juan Basin Public Health (SJBPH) is pleased to announce CoronaDoc, a confidential volunteer program to assist local employers, agencies, schools, and other organizations in educating their employees/members about COVID-19 and best practices for prevention, reducing spread, treatments, upcoming vaccine distribution, and more. The program is focused on short presentations delivered by medical professionals followed by question and answer sessions for participants.

The program is approved and supported by SJBPH but implemented by volunteer administrators and doctors.

Community Compassion outreach

Community Compassion Outreach needs volunteers to donate one home-cooked food item every Saturday for 20 Grab-N-Go Bags distributed to the homeless and food-insecure.

The Coffee & Conversations with the Homeless is held at the front entrance of Durango’s Schneider Park on Roosa Avenue, north of 9th Street Bridge. Individually packaged donations are accepted between Noon and 12:15pm. Two masked volunteers remain to sort donated food in plastic grocery bags to hand out with a bottle of water for 20 individuals who come by our station weekly. Sometimes we have only three items to distribute in each bag when volunteers are not in town. At other times, we may distribute an 8–course meal with desserts.

San Juan Basin Public Health is hiring a Substance Use Prevention Coordinator. This position is responsible for performing community-wide outreach and program promotion in support of contracts. Successful applicants will have experience in organizing community members involvement in local public health initiatives for youth such as substance misuse prevention, tobacco prevention, suicide prevention or other behavioral health/healthy behavior needs. This position collaborates with a diverse set of partners and stakeholders to provide timely education and communication regarding program initiatives; providing trainings and technical assistance to community stakeholders to build their capacity to engage equitably in public health initiatives; provide education directly to youth and caregivers to support building healthy communities; supporting required monthly reporting and program evaluation for funders and internal needs; utilizing data collection tools and methodologies for program analysis; evaluation and monitoring program data and risk indicators to assess conditions; and assisting in writing grant proposals, concept papers and other materials to obtain funding. The qualifications for this position are an associate degree and two years of healthcare, public health or related experience. A bachelor's degree in a related field and experience with youth substance use prevention and/or tobacco use prevention is preferred. This position is scheduled to close 1/3/2021, or until an adequate pool is reached. This hiring range for this position is $17.80 to $25.21. New hires are typically made at or near the minimum of the hiring range; the full wage range is included on the job description. The ideal candidate will have lived or worked in a rural community for 2+ years, have knowledge of food insecurity and food systems, and have experience as a facilitator or consultant. She/he/they must have a bachelor’s degree or 4 years relevant experience. Ability and willingness to travel locally with reliable transportation. The Community Mobilizer is responsible for leading youth and adult community coalitions and workgroups, currently focused on youth substance prevention, in Telluride and surrounding communities. Utilizing the Community That Cares framework and through partnership with local schools, the Mobilizer will lead the implementation of a community youth substance prevention action plan, developing membership in coalition work, and build relationships with community leaders, government officials, educators and other key stakeholders to support the coalition’s goals. Responsibilities also include advocating for community-driven practices and emphasizing capacity building of coalition and community members and engaging local youth to support, grow & lead community initiatives to engage the overall community toward shared goals around youth substance prevention.
