Using Social and Economic Barriers to Health Data to Inform Programming

domo-age-category
When we examine data from social needs screening, what does it tell us? The table above shows the number of social needs by age category in Southwestern Colorado. The graph illustrates that patients between the ages of 19- 64 have more needs on average than both younger and older age groups. The 19-21 year-old group reports the most needs. What do these numbers mean for our region? How can we think about these numbers to support our organizations that are working with these age groups and to create additional needed programs for our community members? All data can be limited by factors like sample size, characteristics of the populations being screened, and lack of context. Yet data can tell us a story about the community members who we serve in our region. We are looking forward to listening to the stories that data from screenings tell us and to collaborating with our community partners to address social and economic barriers to health in Southwestern Colorado.
Note* This Accountable Health Communities Model is supported by the Centers for Medicare and Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $4.51 million with 100 percent funded by CMS/HHS. How Nurses and Medical Assistants Can Foster a
Culture of Immunization in the Practice

Research shows that healthcare professionals are the most trusted source of information for parents when it comes to vaccines for their child. Nurses and medical assistants have a key role to play in improving vaccine acceptance and fostering a culture of immunization in the practice as they are in contact with parents throughout the office visit.

This CE activity features practical strategies to improve vaccination rates in the practice, including how to deliver clear and concise vaccine recommendations and address parents’ frequently asked questions. By highlighting key points before, during, and after a patient’s visit to support vaccine conversations, this presentation will reinforce best practices for improving vaccination rates. Find out how to develop a culture of immunization in your practice.

Audience: Administrators, CHES certified health educators, physicians, LPNs, LVNs, medical assistants, medical students, NPs, nurse technicians, other health educators, pharmacists, PAs, program managers, RNs
