Using Social and Economic Barriers to Health Data to Inform Programming

How do you read the data?

This week, SWCAHEC is posting the AHCM data on social needs and letting YOU read it! AHCM data is aggregate data based on patient screenings in participating clinic settings and healthcare facilities. This data gives us information about the barriers to health faced by community members in Southwestern Colorado. We use this data to create care plans, to coordinate care, and to link with community organizations to increase the wellness of everyone in our region. We also use the data to create new programming and to support existing programs that already serve our region. Please answer the following questions:

1. When you look at the data above, what do you think it means for our region? Your experience will give you a unique perspective on the numbers in the graph above.


2. What do the numbers tell us about social and economic barriers to health in Southwestern Colorado?


3. Given your interpretation of the numbers in the graph, what action would you take to address the community need expressed through the data?

Nurses Leading the Way in Primary Care

Nurses Leading the Way in Primary Care _1_

The 4-day workshop will be held virtually March 2-5, 2021. The workshop will be held from 7:45 am - 4:00 pm each day. Participants are expected to attend all four days of the workshop, as the content is sequential and builds on itself. Participants who complete all hours of the workshop will be eligible to receive nursing contact hours.*

Transgender Health: Comprehensive Approaches in Primary Care


This series is designed to support clinical providers interested in improving care for transgender and gender non-binary patients. Participants will increase their knowledge and confidence in providing gender-affirming care including routine primary care, hormone therapy management, surgical options and supporting mental health.
Sessions include a brief didactic presentation by an expert and discussion of participant-submitted cases. Participants are highly encouraged to submit de-identified patient cases for group discussion and expert consultation.

How Nurses and Medical Assistants Can Foster a
Culture of Immunization in the Practice

Research shows that healthcare professionals are the most trusted source of information for parents when it comes to vaccines for their child. Nurses and medical assistants have a key role to play in improving vaccine acceptance and fostering a culture of immunization in the practice as they are in contact with parents throughout the office visit.

This CE activity features practical strategies to improve vaccination rates in the practice, including how to deliver clear and concise vaccine recommendations and address parents’ frequently asked questions. By highlighting key points before, during, and after a patient’s visit to support vaccine conversations, this presentation will reinforce best practices for improving vaccination rates. Find out how to develop a culture of immunization in your practice.

Audience: Administrators, CHES certified health educators, physicians, LPNs, LVNs, medical assistants, medical students, NPs, nurse technicians, other health educators, pharmacists, PAs, program managers, RNs

The Outreach Coordinator is responsible for increasing community and partner knowledge of Tri-County Health Network’s (TCHNetwork) programs and services, building strategic relationships to identify and address barriers to good health, and conducting a variety of outreach activities. The Outreach Coordinator will also provide direct services to clients. These services include (1) conducting in-home assessments and connecting adults age 60 and over, adults with disabilities, and those living with illness or injury to community resources; (2) oversee the palliative support program and Durable Medical Equipment lending closet, arrange and work with volunteers, and conduct in-home assessments;(3) educating and enrolling eligible individuals and families into relevant public benefit programs (SNAP, Medicaid, CHP+, EOC, LEAP, etc.) and Colorado’s health insurance marketplace; (4) build relationships with external partners and stakeholders to improve our referral network; and (5) assist with other programs, as needed.

The Behavioral Health Care Coordinator (BHCC) links people in San Miguel County to available behavioral health treatment and mental wellness promotion resources. At the same time, the Coordinator advocates on behalf of clients, helps clients navigate the behavioral health system and resources, and coordinates referrals to ensure a streamlined experience for clients. When barriers to care are experienced, the BHCC will assist clients in overcoming obstacles faced in accessing or receiving services needed to maintain well-being and independence through direct services or referrals to other community resources. The BHCC will also provide cross-agency and systems coordination, partner with various behavioral health resources throughout the region, and develop and implement strategies to effectively market and advertise these resources.
