Social Determinants of Health: COVID19 and Beyond


“Understanding what a post-Covid world could look like necessitates an examination of key structural determinants that have contributed to the disproportionate effects of the Covid-19 pandemic on marginalized and other groups, beyond the proximate drivers of the current crisis. Interventions to tackle systematically reproduced conditions of vulnerability would contribute towards a fairer and more sustainable world.”

A recent article in the BMJ highlights the relationship between addressing the health disparities that have become so apparent during the COVID-19 pandemic and taking a social determinant of health approach to medical care delivered now and in the future. According to the article authors, understanding the larger social and economic barriers to health faced by many populations is the first step to reducing the inequities that lead to poor health outcomes.

Healthcare providers who have an expanded view of their patients’ realities are better able to address the many factors that lead to disease and can work to collaborate to provide care that is comprehensive and effective for everyone as we move forward. When healthcare providers examine data about social needs, they gain a picture of how they can support community members who are struggling and increase the wellness of people who live in SW Colorado.

According to the BMJ authors, “the pandemic has highlighted the unequal distribution of power and resources, and people are also using this moment to challenge these inequalities anew.” SWCAHEC and its partners are working to understand existing social and economic barriers to health in our region and collaborating to create services that both address inequalities and create conditions for increased good health now and in the future. As we move forward, we continue to dedicate our efforts to supporting our healthcare providers, community organizations and community members to build strong relationships that improve the health of SW Colorado.

Health Equity: LGBTQ+ Online Course (Mar. 31st-May 19th)

This six-week online learning experience approaches the issue of LGBTQ+ health from multiple perspectives. It will provide information and an opportunity for personal reflection, and ends with an introduction to action steps public health and health care can take to improve health outcomes for LGBTQ+ individuals.

COVID-19 JUST-IN-TIME ECHO FOR PRIMARY CARE

In this ongoing series offered twice a week, an interdisciplinary panel of public health, clinical and pharmacotherapy experts provides information related to COVID-19 and addresses challenges in primary care practice management (such as transitioning to telehealth). Experts answer questions and discuss de-identified patient cases.

Certified Medical Education credits are available for these sessions. Application for CME credit will be filed at the conclusion of this series with the American Academy of Family Physicians. Determination of credit is pending. Credit will be contingent on attendance (please add your name, credentials, affiliation and email address into the Zoom chat during the live session) and completion of a post-series survey.

Perinatal Mental Health: An Interdisciplinary Approach


This ECHO series is designed to support physicians, physician assistants and advance practice nurses who care for pregnant patients in the evaluation and treatment of common mental health conditions presenting in the perinatal period. Facilitated by a multidisciplinary team including obstetrician gynecologists, perinatal psychiatrists, psychologists and social workers, each session will include a brief presentation by an expert followed by discussion of participant-submitted cases. Participating providers will gain confidence and knowledge in addressing the perinatal mental health needs of their patients while building connections with other perinatal healthcare providers.

Transgender Health: Comprehensive Approaches in Primary Care


This series is designed to support clinical providers interested in improving care for transgender and gender non-binary patients. Participants will increase their knowledge and confidence in providing gender-affirming care including routine primary care, hormone therapy management, surgical options and supporting mental health.
Sessions include a brief didactic presentation by an expert and discussion of participant-submitted cases. Participants are highly encouraged to submit de-identified patient cases for group discussion and expert consultation.

The Outreach Coordinator is responsible for increasing community and partner knowledge of Tri-County Health Network’s (TCHNetwork) programs and services, building strategic relationships to identify and address barriers to good health, and conducting a variety of outreach activities. The Outreach Coordinator will also provide direct services to clients. These services include (1) conducting in-home assessments and connecting adults age 60 and over, adults with disabilities, and those living with illness or injury to community resources; (2) oversee the palliative support program and Durable Medical Equipment lending closet, arrange and work with volunteers, and conduct in-home assessments;(3) educating and enrolling eligible individuals and families into relevant public benefit programs (SNAP, Medicaid, CHP+, EOC, LEAP, etc.) and Colorado’s health insurance marketplace; (4) build relationships with external partners and stakeholders to improve our referral network; and (5) assist with other programs, as needed.

The Behavioral Health Care Coordinator (BHCC) links people in San Miguel County to available behavioral health treatment and mental wellness promotion resources. At the same time, the Coordinator advocates on behalf of clients, helps clients navigate the behavioral health system and resources, and coordinates referrals to ensure a streamlined experience for clients. When barriers to care are experienced, the BHCC will assist clients in overcoming obstacles faced in accessing or receiving services needed to maintain well-being and independence through direct services or referrals to other community resources. The BHCC will also provide cross-agency and systems coordination, partner with various behavioral health resources throughout the region, and develop and implement strategies to effectively market and advertise these resources.
