Care Coordination Collaborative

While social needs screening of patients provides data about the barriers to good health that many community members face, the resulting care coordination begins the process of addressing these needs in concrete and immediate ways. SWCAHEC convenes the Care Coordination Collaborative as an opportunity for care coordinators, local community organizations, and healthcare providers to meet and discuss the most pressing issues and needs related to patient care. The collaborative is also committed to promoting clinical and community linkages by educating healthcare providers about resources in our communities that serve their patients. Recently, area care coordinators have voiced the need for a space to case conference and collaborate on meeting the needs of specific patients through collaborative discussions and the pooling of resources. In the upcoming Care Coordination Collaborative, we will discuss this extension of the Collaborative’s agenda and the processes needed to make this opportunity possible. Please join us and add your voice to this discussion so that we can continue to grow and transform to meet the needs of regional care coordination and to further promote the health and wellness of our communities in Southwest Colorado.

We invite you to join us at the next Care Coordination Collaborative Meeting
Wednesday, March 17th, 3:30-5pm.
Contact Heather -[email protected] for details and to sign up.

BID, Visit Durango, and the 5 Star Certification Program have teamed up to create the Durango Mask Marshall program. Marshall Tucker and Miss Kitty are out and about Downtown (as weather permits). These actors are in full old west gear talking with people about the area's mask requirements. Doing this in character creates a calmer atmosphere around the topic of masking.
