How do we use the public health lessons learned in 2020 to improve health for all? The COVID-19 pandemic placed a spotlight on the shocking health disparities that American communities of color have endured for decades and underscored the need for change. Mortality rates linked to COVID-19 for Native Americans, Latinos and Black Americans are approximately double those of White Americans, according to the Centers for Disease Control and Prevention. As the nation recovers, health experts are taking a close look at addressing the health inequities that contributed to this situation and ensuring that it does not happen again.

For this webinar, leaders in public health will discuss key issues that harm health in America’s Indigenous, Black and Latino communities, as well as solutions, including ways to:

  • Transform how we provide health care in ways that address disparities
  • Maximize resources and opportunities for pandemic recovery in communities of color in ways that address the social determinants of health that contribute to inequity
  • Determine how America’s infrastructure - housing, the health care system, transportation, internet access and other factors - can be changed to improve health
  • Address the impact of systemic racism on medical care

San Juan Basin Public Health is hiring a Public Health Nurse assigned to the COVID-19 response team as a Nurse Navigator responsible to monitor confirmed Covid-19 cases and contacts, provide Quarantine or Isolation guidance, refer confirmed cases and contacts, as needed, to medical care and community resources and follow-up with cases and contacts to collect recovery details and data. The Public Health Nurse will collaborate with Case Investigation/Contact Tracing team members and update data tracking systems regularly. Qualifications include licensure as a Registered Nurse in the state of Colorado and one year of nursing experience. Bachelor's degree in nursing (based on assignment). Minimum of two years’ experience in maternal/child health, public health, or mental/behavioral nursing (based on assignment). Or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job such as those listed above. Benefits for exempt employees and non-exempt employees, 80% or more, are Medical Insurance, Dental Insurance, Vision Insurance, Flexible Spending Account, Employee Assistance Program, Wellness Program, Paid Time Off, Retirement Plan, Life Insurance and Supplemental Insurance. This position is scheduled to close May 25, 2021, or until an adequate pool is reached. The hiring range for this position is $24.51 to $30.64 an hour. Overview of the Professional Education Coordinator (Temporary) The Professional Education Coordinator (Temporary) will assist with student communication, enrollment, scheduling, exam support, record keeping and distribution, payment tracking, and program evaluation and reporting. This position may require occasional evening or weekend hours.

Details: This is a full-time, temporary, non-exempt position supporting a staff member on a temporary leave of absence. Worksite wellness program benefits are included. No further benefits are provided. Compensation for this position is $20.00 per hour.

Essential Duties and Responsibilities:
• Enrolls and assists students in online courses
• Communicates with students, Professional Education Manager, and State Board of Nursing via phone, video conferencing, and email to: troubleshoot online access issues; student status, emerging issues, and preceptor sites; and licensure approval
• Tracks online simulation platform experience, grading, scheduling, and record keeping; and payment log tracking
• Assists with student placement into partner database platforms
• Reviews Skills Checklist, attestations, and final exam requirements
• Arranges and supports final exam testing for students online
• Assists with evaluating student experiences, keeps records, collects data, and writes reports on programs administered as requested.

Are you a compassionate individual who cares about public health and community outreach? Do you want to help connect individuals to vital health resources in our community? We are seeking an action-oriented, results driven professional to foster health equity and access.

The Care Coordinator is responsible for increasing community and partner knowledge of Tri-County Health Network’s (TCHNetwork) programs and services, building strategic relationships to identify and address barriers to good health, and conducting a variety of outreach activities. The Care Coordinator will also provide direct services to clients including (1) conducting both phone and in-home outreach and assessments for individuals enrolled in Medicaid; (2) performing in-home assessments and connecting adults age 60 plus and adults with disabilities to community resources; and (3) identifying client barriers to health and connect clients to/enroll clients in relevant programs to promote health.
