Chelsie Begoody has recently joined the SWCAHEC team and we are excited to have her!

Chelsie is from Burnside, AZ, a small community on the Navajo Nation and her clans are Sleepy Rock, Honeycomb Rock, Salt, and Edge Water.

She is currently majoring in Public Health at Fort Lewis College with minors in Nutrition and Environmental Policy and plans on graduating in the fall.

With her passion for nutrition, and her experience in metropolitan

healthcare settings and rural communities, she is a great asset to our team. Welcome!

This free-of-charge training covers a variety of high impact topics related to behavioral health and is designed to support healthcare teams in providing whole-person care for patients across the lifespan. Rather than siloing behavioral health to a singular role in a practice, this training empowers a variety of primary care and specialty clinicians and staff members, including licensed behavioral health providers, to form effective collaborative care teams and provide high-quality care. Adolescent Reproductive Health
For Primary Care Providers

This series offers clinically relevant topics for primary care providers who provide or are looking to build, expand, and improve care to preteens through young adults. Once completed, participants will be able to evaluate and manage common conditions in adolescent reproductive health. Over the course of six sessions, the series will include an overview of adolescent reproductive health issues, best practices in adolescent contraception and common adolescent gynecology concerns.

CME: This ECHO Colorado series will be reviewed and acceptable for up to six credits by the Colorado Academy of Family Physicians.

This position is responsible for performing community-wide outreach and program promotion in support of agency contracts. Successful applicants will have experience in project planning and facilitating work groups and/or coalitions to engage and organize community members' involvement in local public health initiatives. The Communities That Care Coordinator will collaborate with partners and stakeholders to provide timely education and communication regarding program initiatives and provide trainings and technical assistance to community stakeholders to build their capacity to engage in initiatives through understanding of community organizing, sustainability and equity concepts. The incumbent will support required monthly reporting and program evaluation for funders and internal needs, utilize data collection tools and methodologies for program analysis, evaluate and monitor program data and risk indicators to assess conditions and assist in writing grant proposals, concept papers and other materials to obtain funding. The Qualifications for this position are an Associate's degree and two years of healthcare, public health or related experience. A Bachelor's degree and experience with community organizing preferred. Benefits for exempt employees and non-exempt employees, 80% or more, are Medical Insurance, Dental Insurance, Vision Insurance, Flexible Spending Account, Employee Assistance Program, Wellness Program, Paid Time Off, Retirement Plan, Life Insurance and Supplemental Insurance. This position is scheduled to close 6/11/2021, or until an adequate pool is reached. The hiring range for this position is $17.80 to $21.81 an hour. This position supports the continued growth and development of Tri-County Health Network’s (TCHNetwork) diversity, equity, and inclusion programming, community organizing, education & training, and policy & advocacy work. This involves internal capacity building, developing community relationships, coalition building, and ensuring cultural inclusivity. The Director is responsible for overseeing programming that works to empower, engage, educate, organize, and advocate on behalf of our diverse communities including communities that have been socially and economically marginalized. Duties include managing day-to-day program operations; developing field team; establishing relationships with stakeholders; developing and facilitating local coalitions, advancing our DEI agenda, and ensuring program sustainability. Starting salary range is $81,400 – $93,600 based on experience.

Tri County Health Network is Hiring for Care Coordinator!

Are you a compassionate individual who cares about public health and community outreach? Do you want to help connect individuals to vital health resources in our community? We are seeking an action-oriented, results driven professional to foster health equity and access.

The Care Coordinator is responsible for increasing community and partner knowledge of Tri-County Health Network’s (TCHNetwork) programs and services, building strategic relationships to identify and address barriers to good health, and conducting a variety of outreach activities. The Care Coordinator will also provide direct services to clients including (1) conducting both phone and in-home outreach and assessments for individuals enrolled in Medicaid; (2) performing in-home assessments and connecting adults age 60 plus and adults with disabilities to community resources; and (3) identifying client barriers to health and connect clients to/enroll clients in relevant programs to promote health.
