In addition, he or she has a genuine desire to work with the elderly or disabled, reliable transportation, a valid driver's license, a clean driving record, and current automobile insurance and fluent English speaking and writing skills. There will be an emphasis on connecting with and providing education and resources for the Accountable Health Communities Model (AHCM) initiative in western Colorado. The Rural Enrollment Manager will travel frequently, building capacity in small and medium-sized towns for connecting families and individuals to food resources, specifically the Supplemental Nutrition Assistance Program (SNAP, formerly known as food stamps). This role will build relationships and capacity with local regional partners for providing SNAP application screening and assistance, as well as providing these navigation services directly when the circumstances require.
