She has a strong background in education and believes that increased awareness and knowledge is the foundation of finding creative solutions to local problems.

As the Community Program Manager, Christina manages grants and contracts related to health careers, certificate programs and apprenticeships in Southwest Colorado. She grew up in Pagosa Springs and is looking forward to becoming a part of the Durango community and helping improve rural healthcare in the eight county region.

Heather Sorensen, RN, MSN - Education Liaison
Heather has a background in community health, social service and public health nursing in both rural and urban settings. As the Education Liaison, she coordinates the AHEC Scholars Program, housing for student clinical rotations, and pipeline programs for health professionals and high school students including the summer Health Careers Camp.

Health Equity Data Project Manager

The Colorado Trust is seeking responses to a request for qualifications (RFQ) for a Project Manager for the pilot phase of the Health Equity Data & Storytelling Platform Project. This project is a statewide web-based platform, uniquely complemented with human-center technical assistance, that would bring to life stories of residents in communities experiencing inequities, provide compelling data to contextualize and describe those experiences, and serve as a vehicle for sharing examples of progress and innovation that could inspire change.
The project manager will be responsible for all project management and oversight as detailed in the RFQ, and will take a key stewardship role in helping to ensure the community engagement and ownership of the platform. The Colorado Consortium for Prescription Drug Abuse Prevention would love to hear your story. The Consortium is collecting stories from overdose survivors and those who've used naloxone to save a life for an upcoming public awareness campaign to educate more Coloradans about naloxone.

New grant funding allows program to offer scholarships


The Patient Navigator Training Collaborative (PNTC) is excited to announce that the program has been awarded a two-year grant from the Colorado Health Foundation. This funding allows the PNTC to offer financial assistance to individuals and programs, based on need. Full or partial scholarships are available. Applicants must be from Colorado and will still be responsible for a nominal fee of $25 per day of training ($75 for a 3-day Level 1: Health Navigation Fundamentals) to help offset food and administrative costs.

San Juan Basin Public Health is hiring a part time Medical Provider for our Sexual Health Clinic. The provider provides primary healthcare in the areas of reproductive health, including initiation and maintenance of contraceptive methods, breast and cervical cancer screening and sexually transmitted infections. Applicant must be a current Advanced Practice Nurse, Physician’s Assistant, or Medical Doctor. The wage range for this position is $38.13 to $57.17 an hour.   • Bilingual Teletherapist (Contract): We are searching for multiple vilingual (Spanish/English) behavioral health therapists to provide high quality, compassionate and ethical outpatient mental health therapy to adults and to school-aged clients in rural communities in Southwest Colorado using tele-health technology. Therapists, operating as independent contractors, will treat clients via live HIPAA compliant videoconferencing, using the same techniques and approaches as traditional in-person therapy. The Community Partnerships regional associate (RA) supports the regional community partner (CP) to facilitate a strong community partnership process across the multi-county region.

While The Trust is headquartered in Denver, the RA will reside within the boundaries of Region 4 and work out of Delta, Mesa or Montrose counties in southwest Colorado. The RA will be expected to work effectively as a team member and independently, with a high level of self-motivation, integrity and accountability. Suicide Prevention Programs Coordinator/Youth Activities Leader

Responsibilities include implementing the Sources of Strength Suicide Prevention Program at area schools and collaborating with suicide prevention efforts throughout our community, as well as coordinating and supervising group activities for youth ages 7-17.
We are seeking a high-energy candidate who is passionate about advocating for youth in our community and skilled at managing diverse groups of young people. Knowledgeable in use and maintenance of sports and outdoor activity equipment a plus. Ability to communicate effectively and work with a team is essential.
