Health Equity Data Project Manager

The Colorado Trust is seeking responses to a request for qualifications (RFQ) for a Project Manager for the pilot phase of the Health Equity Data & Storytelling Platform Project. This project is a statewide web-based platform, uniquely complemented with human-center technical assistance, that would bring to life stories of residents in communities experiencing inequities, provide compelling data to contextualize and describe those experiences, and serve as a vehicle for sharing examples of progress and innovation that could inspire change.
The project manager will be responsible for all project management and oversight as detailed in the RFQ, and will take a key stewardship role in helping to ensure the community engagement and ownership of the platform. This Medication Safety Learning and Action Network (LAN) Event will feature speakers on the CDC’s Stopping Elderly Accidents, Deaths & Injuries (STEADI) initiative, a QIN-QIO’s de-escalation algorithm for discontinuing medication when burdens outweigh the benefits, and a patient advocate who will share his story on safe medication use. Topics will include approaches to safe prescribing and medication discontinuation practices and a patient perspective on the importance of safe and effective medication management. The Colorado Consortium for Prescription Drug Abuse Prevention would love to hear your story. The Consortium is collecting stories from overdose survivors and those who've used naloxone to save a life for an upcoming public awareness campaign to educate more Coloradans about naloxone.

New grant funding allows program to offer scholarships


The Patient Navigator Training Collaborative (PNTC) is excited to announce that the program has been awarded a two-year grant from the Colorado Health Foundation. This funding allows the PNTC to offer financial assistance to individuals and programs, based on need. Full or partial scholarships are available. Applicants must be from Colorado and will still be responsible for a nominal fee of $25 per day of training ($75 for a 3-day Level 1: Health Navigation Fundamentals) to help offset food and administrative costs.

Participates as a member of the program team to design and implement the individual care plans for program clients in accordance with policies, rules, regulations, and directives which apply. Utilizes DBT skills in addition to a variety of treatment approaches to work with a diverse client population.

San Juan Basin Public Health is hiring a part time Medical Provider for our Sexual Health Clinic. The provider provides primary healthcare in the areas of reproductive health, including initiation and maintenance of contraceptive methods, breast and cervical cancer screening and sexually transmitted infections. Applicant must be a current Advanced Practice Nurse, Physician’s Assistant, or Medical Doctor. The wage range for this position is $38.13 to $57.17 an hour. Although the full salary range for this position is provided, appointments are typically made at or near the range minimum.
